Biological activities

Clopidogrel bisulfate is an ADP receptor antagonist that displays a selective binding on the platelet surface to adenylate cyclase-coupled ADP receptors. Clopidogrel bisulfate is a potent antiplatelet and antithrombotic drug that is used for the prevention of atherothrombotic complications following acute coronary syndromes (ACS) or percutaneous coronary interventions (PCIs). Clopidogrel bisulfate is an inactive prodrug and requires hepatic biotransformation catalyzed by cytochrome P450 isozymes, mainly 3A4, to display anti-aggregation property.[1] Clopidogrel bisulfate in combination with aspirin is the current ??old standard??for reducing cardiovascular events, providing a synergistic platelet inhibition through different platelet activation pathways. Besides, clopidogrel bisulfate induces irreversible modifications of the platelet ADP receptor PY12 mediating inhibition of stimulated adenylyl cyclase activity by ADP. [2] In addition, proton pump inhibitors (PPIs) diminish clopidogrel bisulfate efficacy via CYP2C19 competitive inhibition. [3]
