Biological activities

Mitotane is an adrenalytic drug used in the treatment of adrenal cancer and benign Cushing's syndrome. Mitotane has been associated with cures, long-term remissions, regression of metastases, and increases in survival. The activity of mitotane depends on its metabolism and requires covalent binding to cell macromolecules. Mitotane has a marked suppressive effect on cortisol concentration and cell doubling with IC50 of 20 and 24 nM, respectively. Besides, mitotane suppresses cell growth but does not directly inhibit cortisol synthesis. ^[1] In addition, mitotane is able to reverse in vitro multidrug resistance and can combine with cytotoxic drugs, such as etoposide, doxorubicin, and cisplatin in the treatment of advanced adrenocortical carcinoma. ^[2] Treated with mitotane 47 mg/kg daily for 6 days, the dog serum cortisol levels declines steadily over 48 hours and remains at very low levels throughout the study.^[1]
